Why This Role Is Special

- This is a senior, influential RN position where you'll combine advanced clinical practice with leadership, mentoring, governance, and service development. You'll work across Home Care Packages and broader community services, supporting people to live safely, independently, and with dignity in their own homes.

What You'll Be Doing (and Loving)
- Clinical Excellence & Complex Care
- Lead comprehensive assessments and care planning for clients with complex health, cognitive, functional, and social needs
- Provide expert clinical oversight, consultation, and decision-making in the community setting
- Proactively respond to changes in condition, deterioration, and risk escalation
- Ensure care is evidence-based, person-centred, and aligned with contemporary community nursing standards
Leadership, Mentorship & Influence
- Be a trusted clinical leader across Community Services
- Mentor, coach, and support nurses and care coordinators to build confidence and capability
- Guide best-practice decision-making and model high-quality clinical reasoning
- Act as a go-to expert for complex cases and emerging clinical issues
Service Development & Reform Readiness
- Play a key role in preparing and embedding the Support at Home program
- Contribute to clinical governance, quality improvement, and accreditation readiness
- Support the development and implementation of clinical pathways, protocols, and frameworks
- Partner with operational leaders to ensure sustainable, high-quality service delivery
Connection & Collaboration
- Work closely with GPs, allied health, hospitals, assessors, and external providers
- Build strong relationships with consumers, families, and community stakeholders
- Represent Community Services in governance forums and cross-organisational initiatives
About You
- You're a confident, capable Registered Nurse who thrives in the community space and wants more than "just another role."
- You're clinically sharp, calm under pressure, and genuinely passionate about supporting people to live their best possible lives at home. You enjoy mentoring others, improving systems, and being part of meaningful change.
You'll Bring:
- Current AHPRA registration as a Registered Nurse
- 4+ years' experience in community nursing, home care, or similar settings
- Strong understanding of HCP, CHSP, and Aged Care Quality Standards
- Proven experience supporting reform, quality improvement, or service development
- Excellent communication, organisation, and clinical judgement
Bonus Points For:
- Exposure to Support at Home reform
- Care package budgeting or financial management experience
- Experience across restorative care, reablement, or palliative pathways
