The Role


As a Community Occupational Therapist, you'll provide high-quality, person-centred services to government-funded aged care clients living within the community.


This role will see you travelling throughout Perth's southern metropolitan region, covering from Applecross down to Port Kennedy, with all business-related mileage fully reimbursed.
Working closely with clients, families, caregivers and service providers, you'll deliver evidence-based interventions that promote independence, wellbeing and participation in daily life.
Key responsibilities include:

  • Conducting comprehensive functional and home-based assessments
  • Developing tailored therapy and rehabilitation programs
  • Prescribing assistive technology, equipment and home modifications
  • Supporting clients to maintain independence and safety at home
  • Reviewing and adjusting care plans in line with client goals
  • Providing education and support to clients, families and carers
  • Collaborating with internal and external stakeholders to achieve positive outcomes
  • Maintaining accurate clinical documentation and reports

About You


We're seeking a confident and self-sufficient Occupational Therapist who enjoys building strong client relationships and delivering exceptional customer service.
To be successful, you will have:

  • Bachelor of Occupational Therapy or equivalent qualification
  • Current AHPRA registration
  • Minimum 1 years' experience in community, aged care, rehabilitation or similar settings
  • Experience conducting functional assessments
  • Experience prescribing assistive technology and equipment
  • Experience recommending home modifications
  • Strong clinical reasoning and problem-solving skills
  • Excellent communication and stakeholder engagement abilities
  • Knowledge of wellness, reablement and restorative care principles
  • Current driver's licence and reliable vehicle
