This role is part of a dynamic home care team, offering variety across community clients, collaboration with a multidisciplinary team, and the chance to create meaningful interventions for people living with dementia, chronic conditions, or age-related challenges.

• Deliver individualised occupational therapy services to home care clients.
• Conduct assessments, respond to referrals, and develop tailored therapy programs.
• Work closely with Therapy Assistants and the broader allied health team.
• Collaborate with families, carers, and support networks to promote independence and wellbeing.
• Maintain accurate clinical documentation and ensure compliance with WA aged care standards.
• Contribute to continuous improvement and implement evidence-based, person-centred therapy.

• Full, unrestricted AHPRA registration as an Occupational Therapist.
• Experience in home care, community therapy, or aged care highly regarded.
• Strong communication, relationship-building, and problem-solving skills.
• Ability to manage a varied caseload and adapt to client needs.
• Customer-focused, with the ability to work effectively across a broad range of stakeholders.
• Current NDIS Worker Screening Check or willingness to obtain.
• Commitment to vaccinations and upholding organisational values.

• Full-time or part-time home care hours, supporting clients across Port Kennedy and surrounding areas.
• Salary packaging up to $15,900 per year + $2,650 for meals and entertainment - increase your take-home pay.
