Looking for a short-term contract where you can make a genuine impact within a hospital setting?

We are seeking an experienced Renal Social Worker to join a supportive multidisciplinary team in a regional hospital setting. This is a fantastic opportunity to work with patients managing chronic kidney disease and those requiring ongoing renal treatment, while experiencing one of Australia's most unique and culturally rich regions.

The Opportunity
You'll provide high-quality social work services to renal patients and their families, supporting them through complex health, psychosocial and discharge planning needs.

Key responsibilities include:

• Providing psychosocial assessment and intervention for renal patients
• Supporting patients undergoing dialysis and chronic disease management
• Working closely with medical, nursing and allied health teams
• Assisting with discharge planning and care coordination
• Advocating for patients and connecting them with community supports
• Delivering culturally safe and patient-centred care

About You
To be successful, you will have:

• Social Work qualification eligible for AASW membership
• Hospital-based Social Work experience
• Renal experience highly regarded
• Experience equivalent to an NT Health P2 Social Worker level
• Strong assessment, counselling and case management skills
• Ability to work independently within a multidisciplinary team
• Full Australian working rights (WHV welcome if suitably experienced)
