Home Care Coordinator | Support At Home

Deliver end-to-end case management, ensuring services are tailored, compliant, and aligned to individual client goals and funding requirements.

We are partnering with a well-regarded aged care provider to recruit a Support at Home Care Partner to join their Home Care team in East Perth. This is a community-based role offering autonomy, strong support, and the opportunity to influence care outcomes for older Australians living at home.

The Opportunity
This role is responsible for managing a caseload of Home Care Package clients across varying levels of complexity. You will deliver end-to-end case management, ensuring services are tailored, compliant, and aligned to individual client goals and funding requirements.

Key Responsibilities

  • Manage a portfolio of clients with moderate to complex care needs
  • Conduct initial and ongoing assessments and develop individualised care plans
  • Support clients with budget management, service navigation, and resource allocation
  • Deliver or oversee clinical care where applicable, including wound care and medication management
  • Collaborate with families, allied health professionals, and external providers
  • Provide guidance and support to care staff
  • Ensure compliance with Aged Care Quality Standards and relevant legislation

About You

  • Demonstrated experience in Home Care Package case management
  • Strong understanding of aged care frameworks and Support at Home reforms
  • Ability to work independently and manage a community-based caseload
  • Excellent communication and stakeholder engagement skills
  • Commitment to person-centred, values-driven care

Qualifications

  • Certificate III or IV in Aged Care with strong home care experience, or
  • Enrolled Nurse or Registered Nurse (higher rate applicable)
