I'm currently working on multiple opportunities within the home care / aged care space and am looking to speak with experienced Care Managers, Care Coordinators and Nurse Leaders who understand the Home Care Package environment and enjoy working directly with clients in the community.

These roles sit with established providers across Perth and offer a mix of clinical leadership, care coordination and portfolio management, with opportunities suited to both Registered Nurses and experienced case managers.

Across these roles you may be responsible for:

• Managing a portfolio of Home Care Package clients
• Completing client assessments and developing care plans
• Overseeing care delivery and quality outcomes
• Supporting and guiding support workers and care staff
• Managing client budgets and service utilisation
• Working closely with families and allied health teams
• Responding to changing client needs
• Supporting growth and intake of new clients (some roles)
• Community visits combined with office or hybrid work
• Some of these positions have a strong clinical focus, while others are more coordination and relationship-based, but all require a solid understanding of aged care and home care services.

What I'm Looking For

• Experience working in aged care or home care
• Background as a Care Manager, Care Coordinator or Case Manager
• Nursing experience highly regarded (RN or EN)
• Experience with Home Care Packages
• Strong communication and organisation skills
• Ability to work autonomously and manage a client portfolio
• Confidence working with clients and families
