Clinical Complex Case Manager | Federal Gov Contract

Deliver high-level, trauma-informed case management with complex clients. Federal government contract, hybrid work, competitive rates

Hourly Rate: $67-$76 per hour + super
Contract: 12-month contract
Start: Commencing in approximately 3-4 weeks
Locations: Hobart, Adelaide, Melbourne, Canberra, Sydney, Brisbane, Perth, Townsville, Darwin
Hours: Full-time (37.5 hours per week) 4 days per week may be considered
Work Arrangements: Hybrid and flexible working arrangements to be discussed

About the Role
The Complex Case Manager provides high-level, trauma-informed case management to clients with complex and high-risk presentations. The role involves coordinating care, managing risk, and supporting clients to navigate complex service systems while working closely with internal teams and external providers.

This position suits an experienced clinician who is confident working with complexity, ambiguity and vulnerability.

Key Responsibilities

  • Manage a caseload of clients with complex psychosocial and mental health needs
  • Conduct clinical, psychosocial and risk assessments
  • Develop, implement and review tailored case management and support plans
  • Act as a primary point of contact and escalation point for high-risk matters
  • Coordinate and liaise with health, mental health and community service providers
  • Support clients to navigate systems, services and entitlements
  • Maintain high-quality documentation, case notes and clinical summaries
  • Provide clinical advice and support to colleagues as required

Mandatory Requirements

  • Tertiary qualification (Bachelor's degree or higher) in a relevant health discipline, such as:

-Psychology
-Occupational Therapy (Mental Health Accredited)
-Social Work

  • Minimum 3 years post grad experience
  • Demonstrated experience working with individuals with complex support needs
  • Strong understanding of mental health, disability and psychosocial risk
  • Experience in assessment, triage and case management (including telephone-based work)
  • Excellent communication, documentation and stakeholder engagement skills

Desirable

  • Previous professional experience working with veterans or the Australian Defence Force (ADF)
  • Personal connection to the ADF or veteran community
  • Experience supporting trauma-affected or high-risk client groups
  • Background in mental health, disability, allied health or human services
