This is not your standard Care Manager role.

We're partnering with a progressive, values-led provider undergoing an exciting period of growth and transformation in their Home Care services. As part of a recent restructure, two key leadership opportunities have opened up for Care Managers to take the lead across Perth Metro & Bussleton operations.

This is a role where you won't just manage services, you'll shape them.

The Opportunity

Stepping into a well-supported leadership structure, you will oversee a multidisciplinary home care team delivering high-quality, person-centred care across the metro region.

You'll lead from the front, building capability, driving performance, and creating a culture where both staff and clients thrive.

What You'll Be Doing

• Leading and mentoring a team across community aged care services
• Overseeing care delivery across CHSP and Support at Home programs
• Managing and developing frontline leaders and coordinators
• Driving quality, compliance, and continuous improvement initiatives
• Partnering closely with senior leadership to enhance service outcomes

About You

• You're a confident and grounded leader who understands the realities of home care.
• Proven experience in a Care Manager role within home care or community aged care
• Strong experience leading Care Partners, Case Managers, or Client Coordinators
• Demonstrated people leadership and operational capability
• Passion for delivering meaningful, person-centred care

What's On Offer

• $100,000 - $120,000 + super (depending on experience)
