We're partnering with a well-established aged care provider to recruit a Clinical Care Coordinator for a coastal facility in this highly sought-after Western Australian location.

The Opportunity

• Full-time, Monday to Friday
• Supportive leadership structure, reporting into an experienced Clinical Manager
• Work alongside another CCC in a collaborative environment
• Potential Relocation support + sponsorship available for candidates within Australia

The Role
You'll play a key role in leading clinical care delivery and supporting a high-performing team, with a strong focus on quality, compliance, and resident outcomes.

• Oversee clinical care coordination and delivery
• Support and mentor nursing and care teams
• Ensure compliance with Aged Care Quality Standards
• Manage clinical documentation, care planning & AN-ACC processes
• Act as a key point of contact for families and stakeholders
• Contribute to audits, quality improvement, and clinical governance

About You

• AHPRA Registered Nurse
• 2+ years' experience in a clinical leadership role
• Strong understanding of aged care standards & compliance
• Confident leading, mentoring, and supporting teams
• Experience with audits, accreditation & continuous improvement
• Strong communication and stakeholder engagement skills
