Clinical Nurse Coordinator | Clinical Nurse Manager - Residential Aged Care | Perth Metro

We are currently working with a highly regarded not-for-profit aged care provider seeking a dedicated Clinical Coordinator to join a brand new, modern, purpose-built residential aged care facility located in the East Perth metropolitan area.

This is a fantastic opportunity for a highly experienced Registered Nurse or CCC/ CNM to step into a key leadership role within a values-driven organisation committed to delivering high-quality, person-centred care.

About the Role:

Reporting to the Facility Manager and working alongside another Clinical Nurse Coordinator, you will play a vital role in managing daily clinical operations, mentoring staff, and ensuring compliance with all regulatory and accreditation standards.

Key Responsibilities:

• Oversee the coordination of daily clinical care and care planning
  
• Lead family conferences and maintain positive resident and family relationships
  
• Support AN-ACC assessments and ensure accurate clinical documentation
  
• Contribute to ongoing quality improvement and participate in internal audits
  
• Identify staff training needs and support professional development
  
• Manage clinical stock levels and equipment to support effective care delivery
  
• Respond to clinical incidents and feedback to ensure continuous improvement
  

What We're Looking For:

• Current AHPRA registration as a Registered Nurse
  
• Demonstrated clinical leadership experience, ideally in aged care
  
• Strong understanding of Aged Care Quality Standards and AN-ACC requirements
  
• Ability to lead, mentor and support care teams
  
• Excellent communication and interpersonal skills
  
• Commitment to person-centred care and resident wellbeing
