Are you a Physiotherapist who genuinely loves helping people regain independence, mobility, and confidence? I'm recruiting on behalf of a highly respected aged care and community provider in Harvey, WA, and we're looking for a dedicated physio to join their warm, supportive multidisciplinary team.

This role offers the perfect blend of Transitional Care (TCP) and Community-based therapy, supporting older adults as they move from hospital to home, continue rehabilitation, and maintain their quality of life.

About the Role

Working alongside experienced nurses, OTs and care staff, you'll provide holistic, person-centred physiotherapy to clients recovering from illness, injury, or surgery.

Your work may include:

• Comprehensive physical and functional assessments
  
• Developing tailored treatment & rehabilitation plans
  
• Supporting mobility and independence goals in home, community, and transitional care settings
  
• Collaborating with the MDT to achieve the best clinical outcomes
  
• You'll enjoy autonomy in your day while still having the support of a friendly, established clinical team.

What You'll Bring

• Tertiary qualification in Physiotherapy + AHPRA registration
  
• Strong clinical reasoning and communication skills
  
• Experience or interest in aged care, community rehab, or hospital discharge pathways
  
• A compassionate, client-focused approach
  
• Full working rights in Australia
