Are you a passionate Occupational Therapist looking to make a real difference in older adults' lives? This role supports clients across a 160+ bed facility as well as home and community settings, offering variety, collaboration, and the chance to create meaningful interventions for clients living with dementia, chronic conditions, or age-related challenges. You'll be part of a supportive multidisciplinary team dedicated to high-quality care.
Deliver individualised occupational therapy services to home care clients
Conduct assessments, respond to referrals, and develop tailored therapy programs
Work alongside Therapy Assistants and the broader allied health team
Collaborate with families, carers, and support networks to promote independence and wellbeing
Maintain accurate clinical documentation and comply with WA aged care standards
Contribute to continuous improvement and implement evidence-based, person-centred therapy
Full, unrestricted AHPRA registration as an Occupational Therapist
Experience in home care, community therapy, or aged care highly regarded
Strong communication, relationship-building, and problem-solving skills
Ability to manage a varied caseload and adapt to individual client needs
Customer-focused, with the ability to work effectively across stakeholders
Current NDIS Worker Screening Check or willingness to obtain
Commitment to vaccinations and organisational values
Sign-on bonus upon completion of probation
Full-time or part-time home care hours, supporting clients across Port Kennedy and surrounding Perth areas
