Senior Clinical Care Partner | Registered Nurse

They're growing, they're reform-ready, and they're on the lookout for a confident and compassionate Registered Nurse to join their senior clinical team.

About the Role:

This is a dynamic, leadership-level RN position working across Home Care Packages and community-based services. It's ideal for a nurse who wants to combine hands-on care with clinical leadership, mentoring, and service improvement.

You will:

  • Coordinate a caseload of clients with complex health and social needs

  • Lead and support a team of care staff and clinicians

  • Provide high-quality clinical assessments, planning, and interventions

  • Monitor care plans and adapt as clients' needs evolve

  • Work closely with internal teams and external health providers

  • Ensure compliance with aged care standards and contribute to governance

  • Play a key role in preparing for upcoming Support at Home program changes

About You:

You're a registered nurse with strong clinical judgment, leadership ability, and a genuine passion for community care. You're confident in navigating aged care standards and funding programs and are ready to support both consumers and colleagues through sector reform.

You will have:

  • AHPRA registration as a Registered Nurse

  • Minimum four years' experience in community-based or home care nursing

  • Experience in mentoring and leading clinical or care teams

  • Understanding of HCP, CHSP, and Aged Care Quality Standards

  • Strong communication, organisational and problem-solving skills

  • Confidence in adapting to reform and driving service improvement

Desirable:

  • Knowledge of Support at Home reform

  • Budgeting and care package financial management

  • Experience in restorative or palliative care pathways
