Unique Opportunity | Palliative Care Coordinator

Help drive real change in aged care and be part of something new, with plenty of autonomy and backing from leadership!

Are you a compassionate and experienced Registered Nurse with a strong background in palliative or end-of-life care? This is an exciting opportunity to step into a Palliative Care Coordinator role, supporting the rollout of a new pilot program across two aged care facilities in Perth.


This role is ideal for a clinically confident RN who enjoys both hands-on care and leading broader service initiatives. You'll be supported by a dedicated Care Model Coordinator and work closely with GPs, families, and facility leadership to deliver quality, person-centred care.

Key Responsibilities:


  • Coordinate and facilitate multidisciplinary case conferences

  • Lead care planning and advanced care discussions with residents and families

  • Support and educate clinical and non-clinical staff in palliative care delivery

  • Drive continuous improvement initiatives in line with aged care quality standards

  • Assist in embedding palliative care as part of business-as-usual care delivery

  • Collaborate closely with GPs to streamline care coordination and reduce administrative load

About You:


  • Current AHPRA registration as a Registered Nurse (no restrictions)

  • Minimum 5 years' experience in palliative and end-of-life care

  • Strong communication and leadership skills, with the ability to work independently and as part of a multi-disciplinary team

  • Passionate about person-centred aged care and improving resident outcomes

  • Comfortable working in both clinical and project-focused environments
